Transitional Housing Eligibility Criteria

General Qualifications:
- Person diagnosed with a serious mental illness by a behavioral health professional
- Low income, i.e., SSI, SSDI, etc.
- Good history of ability to maintain the unit in a decent, safe condition
Required Criteria
- Client is stable.
- Client is independent in self administration of medication.
- Client is ambulatory.
- Client is independent in activities of daily living.
Documents to be provided with application:
Referral from case manager & ISP (only portion dealing with housing & recovery goals)
Proof of income or verification of disability benefits and/or employment.
Nazcare currently has eight (8) units which will be used as transitional housing. Each unit has a bedroom, bathroom and living room. The tenants will share a common living room, kitchen and laundry room. Rent will be $300 each/per month including utilities. There is a one time refundable security deposit of $150 per tenant, refunded to the tenant if the room is cleaned and left undamaged. There is a $5 late fee accessed after five days and an eviction notice will be given on the 15th day without payment.
No pets are allowed.
Nazcare offers the convenience of automatic monthly rent withdrawal. Rents are deducted from the accounts on the 4th of each month.
